Wisconsin Licensure Directions

To start your Wisconsin licensure application (early or after graduation) and find ALL the information you need to complete in order to get licensed start at https://dsps.wi.gov/pages/Home.aspx, click on "professions" at the top of the page and scroll down to Advanced Practice Nurse Prescriber and click.  You will find the initial application which is form 2124, the Certification of Master's Degree form (2367), the certification of liability form (2157) and other information as needed.  

Important points to consider with the forms: 

2124-You can apply early, but there will not be activity on the account until you graduate, pass the certification exam, and submit all the required ppw.  Also, you will need to send verification that you completed Pharmacology. The information below came directly from https://dsps.wi.gov/Pages/Professions/APNP/Default.aspx 

     "An applicant must complete and submit proof of at least 45 contact hours in clinical pharmacology/therapeutics including title of course, number of pharmacology hours per course and date of completion (a photocopy of the page of a transcript that contains the 3 credit course is acceptable). This must be completed within 5 years preceding the date of application for the certificate." 

2367-You can not complete this form until after graduation.  Please fill out the top and email to : PImmel@marianuniversity.edu.  If you do not receive confirmation that she sent the form to DSPS within 1 week please send a follow-up email for verification.  

2157-You, do not have to have liability insurance at the time of graduation.  If you are going to be provided liability insurance by your employer (which is usually the case), check that box, sign and return to dsps@wisconsin.gov.
