Submit a request via Sabre Net (redirect to Parchment) for your official transcript to be sent to the national certifying body of your choice (ANCC or AANP). This does not happen automatically and is only initiated by you.
ANCC - Validation of APRN Form - complete the top portion of the form and email to Janet Teske, DNP - Graduate Program Director (jjteske86@marianuniversity.edu). The form has been started for you and is attached below.
AANP - please let clinicalpaperwork@marianuniversity.edu know if there is a downloadable form. We couldn't find one on the website.
To start your Wisconsin licensure application (early or after graduation) and find ALL the information you need to complete in order to get licensed visit the Advanced Practice Nurse Prescriber page on the Department of Safety and Professional Services (DSPS) website.
Review the Credentialing Information for Advanced Practice Nurse Prescriber Certification Applicants and follow all steps.

Form 2367 Certification of Master's Degree will be sent to you via Adobe Sign within 7 days of graduation. The email from Adobe Sign will have a sender's name of, "Marian University Nursing via Adobe Acrobat Sign <adobesign@adobesign.com", with a subject line of, "Marian University Nursing requests your signature on [document title]".
You must have your 10-digit PAR number from the LicensE site to complete the form. You receive the PAR number by starting the application for licensure and paying the fee to DSPS. Do not submit form 2367 without entering the PAR number in the "application number" field.
Once you complete your portion of the form, it will automatically be sent to the Graduate Program Director for signature. The Graduate Program Director will finalize the form and submit it to DSPS on your behalf. DSPS will reject this form if it is received from you. Adobe Sign will send you a copy of the completed form for your records.
Your DEA number allows you to write prescriptions for controlled substances.
Do not start this step until you have passed your AANP or ANCC exam and received your license from the State of Wisconsin.
You will need to complete DEA form 224 (mid level practitioner).

Your NPI number allows you to submit bills/claims to insurance companies for the services you provide.
First you need to make an account with the Centers for Medicare and Medicaid Services (CMS) under the Identity and Access Management System. Once the account is created you will use the same login information for the NPI website.
